The following links will provide the necessary paperwork for a student to particpate in interscholastic sports.
**Assumption of Risk and Athletic Transportation Forms only need to be filled out once in Middle School
and once in High School**
Physical Form, Concussion Waiver, and Insurance Form need to be filled out anually.
