The North Smithfield School Department is hosting the following seasonal flu vaccine clinics.  Both injectable and intranasal vaccine will be available at the clinics.   You may pre-register online at www.thewellcomp.com or you may complete the attached form and bring it with you to the clinic.

There is no charge for individuals with health insurance from Blue Cross Blue Shield of Rhode Island, Medicare Part B, United Healthcare, Tufts, TriCare or Cigna.  A $25 charge will be collected from participants with a different insurance or no insurance.  All participants need to bring their insurance card with them to verify coverage.

Your family members are welcome to participate in the October 5th clinic at Kendall Dean or  on Nov 2nd from 6:45am to 8:00am in the NS High School Library.
